Your Family Counts Home Visiting Program |
The Your Family Counts program (YFC) offers voluntary home visiting services to pregnant women and families with newborns in Alameda County.
YFC includes a team of home visitors: hospital outreach coordinators, infant feeding specialists, public health nurses, family advocates, family counselors and early childhood specialists. Together, the team supports families through the many changes that the prenatal and postpartum periods can bring.
Hospital Outreach Coordinators enroll eligible prenatal clients at a clinic, hospital or home by phone, or in person. Postpartum clients are enrolled in the hospital after delivery. The family is contacted approximately 2-3 days after enrollment to schedule the first home visit. Families are eligible for services up to one year and may be followed longer, depending on psycho-social and child development needs.
Pregnant women receive services such as:
- Connection to community programs and services
- Education around physical and emotional changes during the pregnancy
- Preparation for delivery and the post partum period.
- Anticipatory guidance around infant and baby care.
- Linkage to support services for depression, anxiety, stress, recovery from substance use, trauma domestic violence or loss.
- Child development screening for other children in the home.
- The Kit for New Parents that includes videos, resources and reading material.
In addition to the above services, the postpartum family will receive:
- Physical checks for both mother and baby
- Support with breastfeeding or feeding concerns
- Help with interpreting baby’s behaviors and how to meet their needs
- Education around “parenting style”, infant growth and development.
- Connection to health insurance programs, child care resources and community resources for assistance with postpartum blues/depression/anxiety, stress, recovery from substance abuse, trauma, domestic violence or loss.
