Teen Service agencies provide face-to-face, family centered, early intervention services in a variety of settings for infants born to adolescents and their families. Teens are identified by Hospital Outreach Coordinators at Alta Bates or Highland Hospitals. After the first or second home visit by a public health nurse, infants are assigned to one of two community-based organizations who provide outreach and case management services.

Teen Services Goals are to:
- Identify and meet the pregnant or parenting teen’s needs and goals
- Help monitor and identify developmental needs of child(ren) at home
- Support pregnant and parenting teens to have healthy babies
- Prevent subsequent pregnancies to teen parents
Services offered to teens and their child(ren):
- Coordinating case management services
- Providing assistance to ensure appropriate prenatal, postpartum and well-baby care
- Conducting developmental screening, beginning at 6 months of age
- Making referrals to child development, mental health and substance abuse specialists
- Helping to determine eligibility for other programs (e.g., CAL-LEARN, AFLP, WIC, etc.)
- Securing enrollment and participation in parenting classes and support groups
- Developing and completing a joint “Individual Service Plan,” that documents the teen client’s goals
